2. Complete application and pay DPS fee online @ psp.azdps.gov. then come to our office to be fingerprinted. You must have your IVPP# with you when you come in. You will need to go into your client portal and in "messages", there will be a separate message with this number.

3. Bring your DPS fee ($67) and complete your application in our office.
DPS fee must be in the form of a money order, cashier's check, or business check (these are the only forms of payment DPS will accept).

We mail ALL IVP applications and fingerprints to DPS. We accept cash or card. Our office is open Mon - Sat, 10am - 6pm, no appt needed. We are located at San Tan Village Mall in Gilbert. Use Google Maps for "Madison Reed". There is a corridor next to this store with stairs and an elevator to get you to the offices upstairs. Once on the second floor, go down the left hallway and we are the 3rd door on the right.

Please remember to bring a valid picture ID (DL, state ID, passport, etc).
